ACKNOWLEDGMENT AND ASSUMPTION OF RISK

I, the registered participant (or parent/guardian if participant is under 18 years of age), acknowledge and understand that:

1. Nature of Activity: Goalball is a contact sport that involves physical activity, ball throwing, body contact, and rapid movement in a confined space while wearing eyeshades that limit vision.
2. Inherent Risks: Participation in goalball activities involves inherent risks including but not limited to:
• Collisions with other players, balls, goal posts, or court boundaries
• Falls, trips, or loss of balance due to limited vision while wearing eyeshades
• Muscle strains, sprains, or other physical injuries
• Bruising from contact with the goalball
• Cuts, scrapes, or abrasions from court surfaces
• Aggravation of pre-existing medical conditions
3. Voluntary Participation: My participation in this event is entirely voluntary and I have chosen to participate despite these risks.
4. Physical Condition: I am in sufficient physical condition to participate safely in goalball activities and have no medical conditions that would prevent safe participation, or I have disclosed such conditions to event organizers.

RELEASE AND WAIVER OF LIABILITY

In consideration for being permitted to participate in the NSW Goalball State Championships, I hereby:

1. Release and Discharge the event organizers, NSW Goalball Association, venue owners, officials, volunteers, coaches, other participants, and their respective officers, directors, employees, agents, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury (including death) that may be sustained by me while participating in this event.
2. Waive All Claims that I may have against the Released Parties arising from my participation in this event, whether caused by negligence or otherwise.
3. Agree to Indemnify and hold harmless the Released Parties from any claims, damages, or expenses (including legal fees) arising from my participation in this event.

MEDICAL AUTHORISATION

I authorize event organizers to:

• Provide or arrange for emergency medical treatment if I am unable to give consent
• Contact emergency medical services if necessary
• Contact my designated emergency contact person

Medical Insurance: I confirm that I have adequate medical insurance coverage or accept full financial responsibility for any medical treatment required.

MEDIA RELEASE

I, the registered participant also grant permission for the use of photographs, video recordings, or other media of me taken during this event for promotional purposes related to goalball in NSW.

COMPETITION RULES AND CONDUCT

I agree to:

• Follow all competition rules and referee decisions
• Compete in the spirit of fair play and sportsmanship, in accordance with the Blind Sports Australia Code of Conduct
• Comply with venue rules and safety requirements
• Follow all instructions from event organizers and officials

ACKNOWLEDGMENTS

• I have read this waiver thoroughly and understand its contents
• I understand that this waiver affects my legal rights
• I am agreeing to this agreement voluntarily and without coercion
• If I am under 18, my parent/guardian has also signed below
• This waiver is governed by the laws of New South Wales, Australia
